Sheffield Memory Service &
Complex Geriatric Care
I’m Dr Pravin Jha, a consultant geriatrician offering private memory assessments in Sheffield. Unlike a single-issue memory clinic, I assess memory as part of a whole-person Comprehensive Geriatric Assessment, reviewing mobility, falls risk, medication and general health alongside cognition. No GP referral is required, and I can usually offer an appointment within 24–48 hours at Spire Claremont Hospital, Circle Thornbury Hospital, by home visit, or by video consultation anywhere in the UK. The initial memory assessment costs £800 and includes a full written report.
Private Memory Clinic Sheffield Consultant-Led Memory and Dementia Assessment
If you are concerned about memory problems, confusion, or changes in thinking, a comprehensive memory assessment can provide clarity, diagnosis, and a clear plan.
As a Geriatrician, I do not just focus on memory, but on the whole person — including physical health, medications, mood, and functional ability.
If you are looking for a private memory assessment (diagnosis of dementia), appointments are available within 24–48 hours, including home visits, clinic consultations and video appointments.

Why Choose a Geriatrician for Memory Assessment?
Memory symptoms in older adults can be influenced by many factors, including physical illness, medications, mood, sleep, sensory impairment, frailty and changes in day-to-day function.
As a Consultant Geriatrician, I assess cognition alongside the person’s wider health and circumstances. This includes:
- Potentially reversible contributors to cognitive symptoms
- Medication review, including anticholinergic burden
- Frailty, mobility and falls risk
- Mood and psychological wellbeing
- Functional ability and independence
- Relevant neurological and physical health problems
- Information from a family member or carer, where appropriate
This whole-person approach helps identify contributing factors, clarify the likely cause of symptoms and develop an appropriate, personalised management plan.
👉 This leads to more accurate diagnosis and safer management, especially in older adults with complex health needs.

What Makes This Clinic Different
Each assessment is consultant-led, structured and comprehensive. It combines validated pre-consultation questionnaires with a detailed clinical history, cognitive assessment, medication review, neurological and physical examination, and assessment of everyday function.
Where clinically appropriate, further investigations such as blood tests, ECG or brain imaging can be arranged.
The aim is to provide a careful diagnostic assessment, explain the findings clearly and agree a practical plan for what should happen next.

If Treatment Is Recommended
If the assessment suggests a condition for which medication may be appropriate, the available treatment options will be discussed with you. The choice of treatment depends on the diagnosis, severity of symptoms, other medical conditions, current medications and the balance of potential benefits and side effects.
Medicines such as donepezil, rivastigmine, galantamine or memantine may be appropriate for some people with dementia. These treatments do not cure dementia, but they may help symptoms in appropriately selected patients.
NICE recommends that these medicines are started on the advice of a clinician with appropriate expertise in diagnosing and treating dementia.
Ongoing prescribing and monitoring depend on the medicine prescribed and local NHS prescribing arrangements. In some circumstances your GP may take over prescribing once treatment is established; where this is not possible, private prescriptions can continue.
Follow-up is arranged according to individual clinical need to assess tolerability, response to treatment and any changes in symptoms or function.
